Cat scratch colon during tumor observation prior to endoscopic treatment.

چکیده
A 70-year-old woman was referred to our hospital for treatment of a laterally-spreading tumor (LST) of 40mm diameter, located in the ascending colon. The woman also had chronic renal failure. Before endoscopic treatment, magnifying endoscopy and endoscopic ultrasound (EUS) was scheduled to allow a more precise diagnosis. Initially, chromoendoscopy with indigocarmine, magnifying endoscopy with crystal violet were performed with air insufflation. During the procedure bright-red, linear markings appeared in the ascending colon (Fig. 1 A, B), and the colonic movement became hyperperistaltic. Therefore, the colonoscope was withdrawn immediately and the scheduled EUS was cancelled. The time duration of the observation was 15 minutes. After three weeks, endoscopic submucosal dissection (ESD) for the LST was performed with carbon dioxide insufflation. The LST was successfully removed en bloc. The time duration for the procedure was 60 minutes and cat scratch lesions were not observed throughout the procedure (Fig. 2). Cat scratch colon was first described by McDonnell [1]. Cat scratch colon is a rare condition that occurs spontaneously or partially associated with collagenous colitis, where brightred linear markings appear, mostly on the normal mucosa [1]. Cat scratch colon arising either spontaneously, in the ascending colon, or in diversion colitis, is presumed to be a benign condition secondary to barotraumas, induced by excessive air insufflation during colonoscopy [2-4]. Recently, colonic perforation due to high air pressure was reported, following observation of a cat scratch colon [3]. However, carbon dioxide was rapidly absorbed and was safe during colonoscopy [5]. Because cat scratch colon was not observed during the ESD, with carbon dioxide insufflation, the use of carbon dioxide might have reduced the high pressure that could have lead to perforation. We believe that a large volume of air entering the lumen of the colon during colonoscopy caused this cat scratch colon in the present case.

Cat scratch esophagus.
Cat scratch lesion of the gastrointestinal tract is a rare entity. It was first reported in the colon, in 2007 [1]. A few case reports of cat scratch colon were published in the English literature following the initial description of this entity [2–4]. Recently, a case was described of linear esophageal tears in a man, similar to the colonic cat scratch lesion [5]. We report the second case of ...
متن کامل"Cat scratch" colon in a female patient after a difficult screening colonoscopy.
Figure 1. Figure 1A shows cecal findings from an asymptomatic 59-year-old woman who presented for an initial screening colonoscopy. The remainder of the colonic mucosa was normal. Cecal intubation was achieved in 9 minutes and required repeated loop reduction and abdominal pressure. This image depicts the linear hemorrhagic mucosa found in patients with barotrauma, or “cat scratch” colon. Figur...
متن کامل"Cat Scratch Colon" in a Patient with Ischemic Colitis
"Cat scratch colon" is a gross finding characterized by hemorrhagic mucosal scratches on colonoscopy. It is usually associated with a normal colon and is rarely associated with collagenous colitis. In a previous report, cat scratch colon was noted in the cecum and ascending colon, but has also been observed in the distal transverse colon. The patient in this study was also diagnosed with ischem...
متن کاملThe cat scratch colon sign in a patient with chronic cholestasis.
McDonnell et al. [1], is a rare colonoscopic finding, with only a few articles published in the literature. The patients in these reports have been predominantly female and the finding has been commonly associated with collagenous colitis or diversion colitis. The main proposed causative mechanism is barotrauma [1–3], and until now, the sign has been considered to be a manifestation of certain ...
متن کاملCat Scratch Colon
Over the past few years, we have read several publications regarding the term "cat scratch colon." This neologism was developed to define some bright red linear markings seen in the colonic mucosa that resemble scratches made by a cat. We would like to communicate a recent case attended at our institution.
